ACCUTE / CRISIS STABILIZATION

Paragon's Community Stabilization Program is designed to provide accessible and flexible support that meets people where they are - either in-home, virtually, or in the community. The intensity of services fluctuates based on the individual or family's needs.
The primary goal is to respond with support early in order to avoid the need for costly emergency care or repeated crises. Paragon accomplishes this by making services available when they're first needed - without waiting lists or delays - and always focusing on stabilizing the situation.
Paragon's Community Stabilization Program is designed to be flexible to address the individual's and family's needs, increase resources, and reduce risk factors. Services most commonly provided include:
• Strength-based assessment and care planning
• Skills building using Brief Cognitive Behavioral Therapy (BCBT)
• Family counseling, psychoeducation, coaching, and skills building
• Resource acquisition and coordination to assist with transportation, food, income support, health care, housing, and other basic needs
• Evaluation and prescriptions for medications to treat mental health or addiction related symptoms
• Warm hand-offs to ongoing mental health or substance use treatment
• Assistance transitioning to and from the community when higher intensity services are needed or when returning from higher intensity care

Paragon's Crisis Stabilization teams operate in Adams, Arapahoe, Boulder, Broomfield, Denver, Delta, Elbert, Jefferson, Montrose, Ouray San Miguel, and Summit Counties in Colorado.
